The Sydney Private Hospital welcomes Visiting Medical Officers (VMOs) to apply for Re-Accreditation to our facility. 

Step 1: Please download the and complete the Specialist Re-Accreditation Online Visiting Practitioner Rights Form. Once completed upload to our online Specialist Accreditation Application form below.

Step 2: Please prepare the following documentation for upload

  • Current Curriculum Vitae
  • Photocopy of current AHPRA registration
  • Hand Hygiene Certificate
  • Working with Children Check
  • Photocopy of current professional indemnity insurance certificate of currency showing:
    • Period of Insurance
    • Amount of Cover
    • Category of Insurance
    • Copy of Radiation License – if you intend to operate x-ray equipment.
