Your doctor or your doctor’s receptionist will advise us of the date of your admission to the hospital and the proposed date of your operation/procedure.

Pre-Admission “Patient Information” Form

There is some essential information that is required for your safety and for other functions such as reporting to the Health Department and your private health fund.  This is called a Pre-Admission Form, part of which will be completed by the staff at your doctor’s office.
