The Sydney Private Hospital welcomes Visiting Medical Officers (VMOs) to apply for Re-Accreditation to our facility.

Step 1: Please download the following form: Specialist Re-Accreditation then upload to our online Specialist Accreditation Re-Application portal below.

Step 2: Please prepare the following documentation for upload

• Photo ID

• Police Check

• AHPRA Registration

• Working with Children Check

• Photocopy of current professional indemnity insurance certificate of currency showing:
  – Period of Insurance
  – Amount of Cover
  – Category of Insurance

Copy of Radiation License – if you intend to operate x-ray equipment.
