Your doctor or your doctor’s receptionist will advise us of the date of your admission to the hospital and the proposed date of your operation/procedure and most of the essential information we require.
You will be asked to complete the “Patient Information Form” which will advise us of your medical history.  This information can be posted to the hospital before your admission date or sent by fax to 02 9716 3537 or you can deliver it in person to the hospital reception.

One of our admission staff will contact you prior to your admission date to confirm details.

Having all the information we need prior to your admission will make it easier for you on the day of admission to the hospital.

It is important to note that surgery schedules may change which will alter the arrival time given to you by your doctor.  Please follow any instructions given by the reception staff at The NSW Eye Centre.  This may save you unnecessary waiting time at the hospital.

   

Accounts
Patients holding top level health fund cover are requested to provide evidence of health fund coverage and sign a release form so that the hospital may claim on your behalf   (most health funds issue a plastic card).

Other patients who have chosen not to have full health fund cover are required to pay the estimated balances not covered by their health fund on admission.  Some health funds do not provide a ‘no gaps’ cover and therefore you will be required to pay a balance.  This will be discussed prior to your admission to the hospital.
NB:  Be sure your health fund membership is up to date, and discuss your hospitalisation with your health fund Claims Department if you are in any doubt as to what benefits may apply.

Self-funded patients will be required to pay the estimated total cost of hospitalisation on admission.

Patients whose accounts are subject to “Third Party” or “Workers Compensation” claims must have verifiable evidence that liability has been accepted by the insurance company for their hospitalisation.

Doctor Fees
All doctors including surgeons, pathologists and anaesthetists are private practitioners and will bill you individually and separately for their services.
